The Prosper Club Membership Benefits:

This agreement is entered into as of ____________ (date), between Massage and Prosper LLC and ________________ (full name).

 

This agreement is being entered into for the purpose of ________________(name), to purchase and receive no less than one (1) massage every month at membership pricing. This price per massage cannot be combined with any other discounts, coupons, or specials unless specified by Massage and Prosper.

____The card provided will be charged $100 every 1st of the month. Remaining balance, tax and gratuity will be due after each session. 

​

___Membership agreement can be canceled at any time after 3 months without penalty. 

​

___ If you miss a massage for the month, the massage will roll over into the next month. Upon cancellation or termination of membership, all unredeemed massages will be forfeited.

​

___ Massage hours cannot be combined to make a 2(+) hour massage.

​

___ There is a 24 hour cancellation policy for any massages scheduled. You may cancel your appointment without charge up to 24 hours preceding your appointment time. 

​

___ Same day cancellations will be charged 50% of the membership price ($40). If you do not call/text to cancel your appointment or do not show up for your appointment, you will be charged full price for the scheduled service. Such fees must be paid before any future massages are scheduled.  Monthly membership massage(s) will continue to be charged to your card and roll over. 

​

___ We allow a limited window of 30 minutes if you are running late. Your session may be limited to the time left or a $30 fee to reschedule. 

​

___ If you are planning on going out of town or need to take a break from the massage club, text/email us at any time to put your account on hold and schedule a commencement date of billing.

​

___ If you would like to split the massage club contract with your spouse or SO / child / family member/ friend (such as mother, sister, brother) you are free to do so up to two times a month. You may share the membership pricing for initial massage or the 15% discount for all additional services twice.

​

___ Massage and Prosper reserves the right to cancel your membership for any reason. If the office should cancel your contract, you will not be reimbursed for any received massages and will not be held accountable for the rest of the contract.

​

___I authorize Massage and Prosper, LLC to charge the card I have specified regarding monthly dues and possible fees in accordance with the membership agreement above. Monthly dues will be withdrawn on or after the same day of each month. I also authorize Massage and Prosper, LLC to charge my credit card on file in lieu of presenting it.

We agree to sell and you agree to purchase the membership, good and services described herein. You agree to pay us for the membership, goods and services according to the payment schedule below. Your signature below indicates your agreement to be bound by the terms and conditions of this agreement.
